Requirements for Transmission/ letters of Administration/ Probate and Change of Name

(A) Transmission/Letters of Administration/Probate

1.

Two (2) copies of Letters of Administration/probate that MUST contain the listing of shares of the deceased. An amended copy of letters of administration/probate is required where one of the administrators/executors is dead.

  • •The letters of administration MUST bear ALL the names of the deceased.
  • •In case of probate (photocopy and original will required).
  • •The administrator(s) is/are expected to be physically present for proper identification.
  • •Means of identification: administrator to produce either (a) Birth certificate (b) marriage certificate (c) Driver's license or (d) International passport (original for sighting).
2.

Two (2) copies of Death certificate (either from the Hospital or National population commission).

3.

Two (2) copies of the Gazette/Newspaper publication.

4.

Bankers confirmation of signature of Administrator(s) opened in the Estate account with the Administrator(s) passport(s) duly endorsed on it. (Kindly note the following: (1) The Estate account must have been in operation for a minimum of six months. (2) There must be no disclaimer clause and (3) signed by two authorized bank signatories with their names/page numbers on the bank letter head.

5.

Letter of request for transmission by the Administrator(s)/Executor(s).

6.

Original Share Certificates for endorsement.

7.

Shareholder data form to be duly completed by administrator(s).

8.

Original copies of all the required documents for sighting only.

9.

Payment of the specified sum for confirmation of letters of administration/probate.

(B) Registration for Change of Name

•Introduction letter.
•Photocopy of marriage certificate and original for sighting.
•Original Banker's confirmation of signature with passport duly endorsed on it. (Kindly note the following: (1) The account must have been in operation for a minimum of six months. (2) There must be no disclaimer clause and (3) signed by two authorized bank signatories with their names/page numbers on the bank letter head.
•Original sworn affidavit.
•Original Share Certificates for endorsement (if any).
•Photocopy of Newspaper publication and original for sighting.
•Means of identification and original for sighting.
•Shareholder data form to be duly completed.
